A psychiatric evaluation is a comprehensive assessment with one of our psychiatric nurse practitioners to better understand your symptoms, concerns, health history, and goals so we can recommend the right treatment approach. Many people seek an evaluation when they're experiencing challenges such as anxiety, depression, ADHD, mood changes, sleep concerns, or other symptoms that are affecting daily life.
Our psychiatric nurse practitioners take a whole-person approach to care. Along with understanding your mental health symptoms, we consider the many factors that influence well-being, including your physical health, relationships, family, life experiences, and daily functioning. From this perspective, we'll create a personalized treatment plan together that may include medication, therapy, lifestyle changes, referrals to other specialists, or a combination of approaches.
If medication is part of your treatment plan, we'll provide ongoing medication management to help ensure it's working safely and effectively. We'll regularly check in on your progress, answer your questions, and monitor for side effects. Our goal is to help you find the right balance of treatments to support your long-term health and well-being.

Get matched with a clinician.
Within 7 business days, our clinicians will review the information you shared on your Appointment Request Form and confirm if we can offer a strong clinician match to meet your needs. If it's been 7 business days and you haven't heard from us via email, please check your spam folder, or contact us at 503-729-1380.
If we can offer a strong clinician match, our support team will contact you to prepare for your Intake Session by completing the required forms below.
If we are unable to offer a strong clinician match at this time, we'll connect you with one of our trusted community partners.

Using our Appointment Request Form, tell us a little about yourself, your needs, and the support you're looking for. Watch for a confirmation email that your request was received and is under review.

Before you schedule your Intake Session, please complete the three forms below. Each link opens a new tab so be sure you've completed all three.

Complete your intake session.
Once we've confirmed a clinician match and have received your required forms, our support team will contact you by phone to schedule your first Intake Session.
This session is centered around understanding and diagnosing your care needs. We’ll use this session to either build a treatment plan tailored to your unique needs, or in the rare case that we discover we're unable to meet your needs, we'll connect you with one of our trusted community partners.
