Request care
Using our Appointment Request Form, tell us a little about yourself, your needs, and the support you're looking for. Watch for a confirmation email that your request was received and is under review. ​

Care match
Within 7 business days, our clinicians will review the information you shared on your Appointment Request Form and confirm if we can offer a strong clinician, class, group, or program match to meet your needs. If it's been 7 business days and you haven't heard from us via email, please check your spam folder, or contact us at 503-729-1380.

If we can offer a strong clinician and class/group/program match, our support team will contact you to prepare for your Intake Session by completing the required forms below.

If we are unable to offer a strong match at this time, we'll connect you with one of our trusted community partners.

Complete required forms
Before your scheduled Intake Session, please complete the three forms below. Each link opens a new tab so be sure you've completed all three.
2. Personal History Questionnaire (PHQ). Select one based on the age of the patient:​

3. Professional Disclosure Statement—This form is only required for patients who are requesting Therapy with an LMT, LMFT, and registered associate clinicians.

Complete your intake session
Once we've confirmed a clinician match and have received your required forms, our support team will contact you by phone to schedule an Intake Session.
This session is centered around understanding and diagnosing your care needs. We’ll use this session to either confirm enrollment in the class, group, or program you've requested, or in the unique case that we discover we're unable to meet your needs, we'll connect you with one of our trusted community partners.

If we’re able to offer a strong class, group, or program match for you, your next step is to begin participation with a plan designed to support your goals, growth, and well-being.
